How does right-to-left design change an app?

Everything mirrors: layout, navigation, icons that imply direction, and the order in which the eye reads a screen. Text alone is the small part.

What decides it

The details that get missed are back arrows, progress indicators and carousels, all of which point the wrong way when only the words are translated. Numbers and Latin brand names stay left-to-right inside right-to-left text, which is the part that has to be handled rather than flipped.

The part that gets skipped

Arabic is not a translation job. A right-to-left screen reads in a different order, so the layout changes and not only the words.

Phone-first is not a trend here, it is the default. A store that works badly on a phone works badly.
